Downloadable Intake FormsYou will need to fill out and sign some forms during your first visit. Some forms are available here to download, such as your Medical History Form. If you have been diagnosed with temporomandibular dysfunction (TMD), you may download our TMJ History Form. These two forms will be reviewed by Dr. Fulton during your initial consultation. Filling out the medical history form in advance can save time during your first visit. Prior to starting treatment additional forms will need to be signed. You may choose to review these forms online: Informed Consent, Notice of Privacy Practices and Federal Truth and Lending forms.
