Transfer Patients

The Hampton Roads region is home to many branches of our nation’s military. Dr. Catherine Oden Fulton is happy to accommodate military patients and others who transfer in and out of her orthodontic practice. The American Association of Orthodontists (AAO) has guidelines that orthodontists follow to help patient transitions to take place smoothly.

It is helpful, but not absolutely necessary, for you as a transferring patient to give us the following:

  • AAO Transfer Form, which the originating orthodontist completes.
  • Former and/or current records (x-rays, photographs, and/or models)
  • To expedite the transfer of your case, you may download a medical history from and a release of records form from our Downloadable Intake Forms page. Once completed, the release of records form may be sent to your former orthodontist or you may bring it to our office and we will send it. Of course, you can also get the release form from one of Dr. Fulton's team members."
  • Other downloadable initial consultation forms

Whenever possible, Dr. Fulton will utilize the original orthodontist’s braces and complete the original treatment plan. This is often the most efficient and cost effective way to handle a transfer case. Dr Fulton attends periodic training courses for a variety of bracket types, including edgewise and self-ligating (Damon, GAC, Speed) braces and Invisalign. She also utilizes the Herbst appliance, versus headgear for Class II correction.
